A. Manuscript : Check from beginning to the end: in the focus group

• we asked pts to report any further possible domains or concerns that we didn't include in this questionnaire
• they didn't report or we did find any further domains

B. We know for sure about there are 3 broad views of ADL perceptives exist

• draw out the best concepts of definitions from these 10 articles and start our own definitions
• remember to put citations

C. Things to improve

• try to find a way to solve the problems ASAP, ex: report back or seek to Prof. Hsieh's help
• knowing the problems exist, face it

Difficulty in summarizing the ADL perspectives

The difficulties that I have encountered in summarizing the definitions of ADL - 3 perspectives

1. 在這10篇研究，並不是每篇都有清楚的將他們所謂的 ADL -perspectives 定義寫出來
2. 有些 studies 是在探討 modes of measurement, 例如 self- reporting vs. actual performance, 所以並沒有寫出直接相關的定義, 所以當讀完一篇 study 後 ....還得反覆想到底那些是我們要的或有關於這 3 個 ADL perspectives 的部份.....在這部份花了不少時間....有點抓不到重點的感覺 .. ..

A. For the Medline database :

1. 現在 search stratgy (key words: RCT/ stroke/ ADL and/or IADL/yr "2005-2009) 已完成
2. 現在已將 final results exported to Endnotes 做 screening for the yr 2007 -2009
3. yr 2005-2006 已經完成 screening and summary
4. A total of 374 articles meet the criteria ; of these 374 articles, 296 articles have been screened, still have 78 articles to go; of these 296 articles, 43 articles need to be summarized.

B. For the OT seeker database:

1. 現在 search stratgy (key words: RCT/ stroke/ ADL and/or IADL/yr "2005-2009) 大致已完成，但有些 minor changes have made, 所以還需要再重新 screening the articles again
2. 之前已有完成一些 year 2005-2006 的 screening & summary
3. 因為 OT seeker 的 search interface 比較特殊, (得多花一點點時間)

• To import references, follow the instructions below:
  1. Perform a search on OTseeker.
  2. Select the articles that you wish to save to a reference management system.
  3. Select "View detailed selected results" at the bottom of the page to show the detailed search results.
  4. At the bottom of the Detailed Search Results page, choose the email format "Tagged text format" and email the results to yourself.
  5. Save the text from the body of the e-mail as a text file (*.txt).
  6. Import the saved text file into your reference management system (eg Endnote) using reference manager (RIS) as the filter

* A total of 199 articles meet the criteria ; of these 199 articles, 20 articles have been summarized, and still have 179 articles to go.

C. For the CINAHL database:

1. 現在還在 search strategy 的階段，已完成 key words : RCT & stroke 的 search, 今天 10/21會 開始 search " ADL & IADL " & 其他的 criteria (ex: limited to human & english, etc.)
2. 10 /21 完成了 search strategy, for the whole set of search strategy, please click here , thank you!
3. A total of 583 articles meet the criteria; none of these have been screened and summarized.

Questions:

1. Some studies used stroke just as one of thier study's criteria. For example: The study included the patients who have been diagnosed with stroke, diabetes, and arthritis. Just wonder if we are going to include this in our study.
2. Some studies only used "13 motor items from the FIM instrument" (not the whole set of FIM) as thier primary or secondary outcome measure. Are we going to include this as well?

Difficulty:

• These two days I spent a lot of time to get familiar with the CINAHL database interface. I would say their "save/ retrieve search history" functions are not user-friendly. I lost the data again and again and have to repeat the steps (re-type/re-entrance /re-search) many times. It takes up a lot of time that sometimes make me feel so frustrated. Anyway, I will try my best to figure out how to save my data and also my time as well. Hopefully, I will speed up my workflow and make more progress soon.

Reflection

這幾個星期陸續有加拿大大學及研究所 OT 的同學來拜訪，自己一方面很高興這些朋友願意花錢來探望我這朋友，也感謝台北捷運系統的便捷，也慶幸自己在這幾個月已大致適應了北部生活，才能盡地主之誼將美麗的北台灣介紹給這些所謂的 CBC (Chinese-born Canadian) 的朋友 欣賞。

但另一方面愁悵的是這些同學工作了幾年 (大學同學已工作 3年; OT 同學 工作 將滿 1年)，已經存夠錢出國遊玩，尤其是我這位 OT 同學第一年已有14天 有薪假 (14天還不包括 weekend)。這次她們共同問我一模一樣的問題....." 後不後悔回來台灣 ? "

這個問題一直圍繞在我腦裡直到這個 weekend 我有機會帶這位大學同學重遊陽明山，我才頓時豁然開朗。依然記得在我非常小的時候 (打電話求證我媽, 她說當時的我才6 歲)，在陽明山花鐘前我們全家照了一張全家福，當時的我是如此的小，綁著兩個馬尾，我媽還穿著當時最流行的喇叭褲，我爸是如此的年輕瀟灑，當時的我看他們是如此的高，如此的健壯。
在那剎那我明白回來的意義了，事隔 30年，再一次重遊陽明山，在花鐘前心裡悸動感受特別深，30 年了，我已不再是那永遠需要他們保護照顧的小女孩。然而我的父母卻日益變老，不再是照片中那高挺健壯的模樣。以前是他們在照顧保護我，現在是我該反哺回饋照顧保護他們的時候了。

當初去加拿大雖不是出於自願，卻也是讓我得到成長最多最快的時期，回來是我自己的決擇及決定，我不後悔也更加堅定了自己的心。接下來別無所求只期望能做到父母能以我為榮。

Manuscript 所遇到的困難

Methods section:
1. 請說明病人的代表性，是所有病人或方便樣本？
---> a convenience sample of patients with stroke
---> later on could be stated this as one of our study limitations

2. 過程是由幾位研究人員或專家執行？
---> Consult with 5 experts and clinical practitioners in rehabilitation

3. 如何選擇及分類的？即判斷標準為何？是否只要有一個病人提到的，都會放進去，還是要數個病人都提到的 domain 才會放入？.
---> inputted from the stroke pts and reviewed and summarized by experts
---> field testing, comments were reviewed and changes made until no more new domains came up or no substatial changes were suggested

Reflection

台中之行之心得:

1. 此次和筱文面對面逐一解釋我的量表，是一個非常好的學習機會，尤其筱文在收案方面已有3年經驗，在加上她本人有同時使用多個量表的經驗，所以也提供了不少寶貴意見。
2. 在解釋量表的過程中，更釐清了自己的思路，也更了解這量份表的目的，並能清楚的將此目的傳達給將使用者。
3. 量表方面: 在某些項目裡面呈現了 "城鄉之間的不同" ，例如搭乘交通工具方面，以及休閒活動方面。一些細節 (要修改的部份) 會在修改好之後，在 post 上來。
