The Barthel Index (BI) has been identified because it has matched the conceptually defined of the aspect of actual performance. The BI is wildly used measurement to assess patients’ actual performance in ADL. Among all other ADL measures, the BI has been regarded as the best in terms of its inter-rater reliability, convergent validity, simplicity, and ease of scoring. Because of these, the BI has become the common measurement used by both clinicians and researchers in the field of rehabilitation.
Despite the growing consensus that it is important to measure not only the ADL performance of patients but also their perceived difficulty and ADL ability, no assessment tool can assess all three ADL aspects simultaneously. To address this gap, we sought to build on existing BI and to develop a comprehensive measure we called the 3-asepct Barthel Index. In addition, we also examine the internal consistency and test-retest and inter-rater reliability of the 3-aspect Barthel Index to valid its utility in patients with stroke.

The Barthel Index (BI) has been widely used in the field of rehabilitation and regarded as the best in terms of its inter-rater reliability, convergent validity, simplicity, and ease of scoring.11, 12, 13 While the BI was adopted to assess patients’ actual performance in ADL, there has not been one to assess all three ADL aspects simultaneously. Therefore, our study of purpose was to build on existing BI to develop a comprehensive measure we called the 3-asepct Barthel Index. In addition, we also examine the internal consistency and test-retest and inter-rater reliability of the 3-aspect Barthel Index to valid its utility in patients with stroke.
